The Intake Process

An assessment is needed to begin receiving services. Assessments can be completed by your medical service provider or by our team of sub-county nurses. If you do not have a recent medical assessment, our sub-county nurse will contact you and schedule an in-person interview in the comfort of your own home. Our intake assessments are designed to determine the care services that are appropriate for your needs and long-term health goals.
